A workable structure runs to six parts. Purpose, in one sentence. Who it applies to, including contractors where relevant. The position, meaning what the organisation expects or will not accept. What to do, including who to contact. What happens if it is not followed. And the version, owner and review date.
Two things are worth keeping out. Legislation quoted at length, which dates the document. And procedure detail that changes often, which forces a reissue and a fresh acknowledgement round.
Consistency matters far more than eloquence here. People learn where to look in a document and stop reading the parts they do not need.
