At hiring. A candidate discloses a condition at interview and is told the role would probably be too demanding, without any discussion of what the role actually requires or what could be adjusted.
During employment. An employee returning from a period of leave finds their responsibilities have been redistributed and not returned, described as taking the pressure off.
At exit. A restructure selects the person whose attendance record includes disability-related absence, with that absence treated as a performance factor.
Each of the three was probably well intentioned by somebody at the time. Intention is not the test that gets applied afterwards.
