What happens next is where injuries and incidents come from. A worker improvises, manages, and says nothing, because reporting feels like admitting they could not cope.

Training worth running says the opposite explicitly. If the equipment or the second person is not there, the task does not proceed as planned, and that gets reported rather than worked around. It also has to be true, which means the organisation must respond to those reports rather than treating them as a complaint.
