Employee training is planned around delivery. Content, format, who runs it, when it happens. The evidence side is treated as administration and it is a separate project.
The two come apart in familiar ways. A good session delivered in a room with a sign-in sheet nobody scanned. A toolbox talk that happened and was never recorded. An external course completed with the certificate sitting in somebody’s inbox. Content updated without recording which version people saw.
None of that is a training failure. It is a record failure, and it produces the same result if anybody asks.
What closes it is unremarkable. Every session assigned to named people. Completion recorded against the person, not the session. The content version stamped. And external certificates captured centrally rather than personally.
