How to collect employee feedback is usually decided by picking a survey tool, and two design choices matter more than the tool.
Frequency first. An annual survey produces a result that takes weeks to analyse, arrives months after the experience it describes, and is stale before any action lands. Shorter and more often beats long and annual, provided something visibly happens in between.
Question design comes second. Long agree-or-disagree batteries produce a score and almost no information. A handful of specific questions and one open text field will usually produce more usable material than forty rated statements ever do.
Only ask about things you are actually willing to change. Asking about pay when the pay structure is fixed teaches people the whole exercise is decorative.
