How to gather staff feedback is usually treated as a question of method. Who asks matters at least as much.
A direct manager asking gets a filtered answer, particularly about anything involving them. A senior leader gets the version people think is expected of them. HR gets more candour on process and rather less on individuals. Somebody external gets the most candour and the least context.
None of those is wrong. The mistake is treating any one of them as the truth, and then being surprised when another channel says something quite different.
Use more than one channel deliberately. Manager conversations for day to day work. An anonymous route for anything about conduct or about a manager. Occasional external facilitation where the subject is genuinely difficult.
