HR processes get documented individually and fail collectively, because the risk sits at the joins rather than inside any one of them.
Recruitment hands to onboarding. Onboarding hands to payroll and to the manager. A role change hands to payroll, to access and to training assignment. Termination hands to payroll, to IT, to assets and to records retention.
Each handoff is a point where something can be dropped without anybody noticing, because the sending process considers itself finished and the receiving one never knew it was owed anything.
Mapping is worth doing once. For each handoff, write down what passes, who is responsible for confirming it arrived, and what happens if it does not.
That exercise usually finds two or three gaps that have existed for years.
