Risk Aware Culture
A risk aware culture is claimed in far more organisations than it exists in, and the claim is easy to test.
Watch what happens when someone raises a problem early. In an organisation that is genuinely risk aware, the response is questions and support. In one that is not, the response is irritation at the timing, or a search for who is at fault, and the next person waits longer.
Other visible signs. Near misses get reported at a rate that looks high. People say they do not know rather than guessing. Decisions record what was uncertain at the time. Bad news travels up faster than good news.
None of that is produced by a policy. It is produced by what senior people do the first few times it is tested.
