A requirement is applied to everyone. It looks even-handed. In practice a person with a disability cannot comply with it, and the requirement is not reasonable in the circumstances.
Common forms include attendance rules that leave no room for treatment appointments, communication that exists only in one format, recruitment processes with timed online components unrelated to the job, uniform requirements incompatible with a medical device, and meeting or event venues without accessible access.
Nobody has to have intended anything. The question is whether the requirement is reasonable, and whether an adjustment would have removed the disadvantage.
