What is incident reporting actually for? Organisations answer that it creates a record, and the record is a by-product rather than the point.
The purpose is to locate the control that failed while it is still cheap. Every incident is a control failing under real conditions, and it is the only test of your controls you will ever get for free.
That framing changes what you do with a report. If the outcome is a conversation with the worker about being more careful, you have treated the person as the failure, and the control is still sitting there waiting for the next person.
The question worth asking on every report is narrower. Which control was supposed to prevent this, and why did it not work?
