What is incident reporting and investigation, treated as two stages rather than one phrase? They have different owners, different timeframes and different purposes.
Reporting happens immediately, by whoever was there. It captures facts while they are accurate. Speed matters more than completeness, and it should take two minutes.
Investigation happens afterwards, by someone with time and enough distance to be objective. It establishes why, using the facts already captured at the scene.
Collapsing them causes a specific failure. When the person reporting is also expected to explain the cause, on the spot, under pressure, the answer is almost always human error, because that is the fastest available explanation.
Separate them and investigations start finding what was wrong with the task, the equipment or the staffing.
