A form captures. A system does four further things.
Routes it. To the right person automatically, based on type and severity, without someone remembering to forward an email.
Tracks the action. With an owner and a due date, so corrective actions close rather than drift.
Links to the risk register. So an incident points back at the control that failed.
Reports patterns. By site, team and hazard type, which is where the useful information lives and which a spreadsheet will not surface.
The third is the one that separates a genuine system from a better form, and it is worth testing in any demonstration.
