Surprises. A concern raised for the first time in writing. Fix by requiring anything in a review to have been raised at the time.
Recency. Twelve months assessed on two. Fix with notes kept through the year.
Everyone in the middle. Fix with calibration and a scale with no midpoint.
Nothing happens afterwards. Fix by requiring one or two agreed actions with dates.
Completion treated as success. Fix by reporting on whether anything changed rather than on how many forms were submitted.
The last one is the reason all of the others persist year after year, because nobody is measuring the thing that would show they are still there.
